The women in the study, aged from 15 to 45, were compared with 47, women of a similar age who had not lost a baby during pregnancy, of whom 1, 2. They identified the drug with the highest risk of miscarriage as Diclofenac when used on its own, and the lowest as rofecoxib, which was taken out of use in because of safety concerns.
The authors concluded: "Women who were exposed to any type and dosage of non-aspirin NSAID during early pregnancy were more likely to have a spontaneous abortion. If a pregnant woman does need to take any analgesia, then paracetamol would be appropriate," said Janey Fyle, the RCM's professional policy adviser. Jane Bass, the RPS's women's health spokeswoman, said: "This study reinforces current advice that women should avoid ibuprofen and other non-steroidal medicines in pregnancy. To more safely relieve aches and pains during pregnancy, use acetaminophen Tylenol instead.
Note that no medication is considered percent safe. Tylenol is generally recommended for pain in all stages of pregnancy — first, second, and third trimester. Even so, you should still consult your doctor first. And if possible, limit use of the pain reliever and take the lowest dose. For example, use a heating pad or a warm compress to relieve backaches, headaches, leg cramps, and pelvic pain.
You can also alternate between heat therapy and cold therapy to reduce inflammation. Pregnancy pain can range from mild to severe. For moderate to severe pain, a safer pain reliever can reduce inflammation and help you feel better — but always consult your doctor first. A further limitation is that, though the researchers aimed to adjust their results for a wide range of medical conditions that could have affected the observed relationship between NSAID use and miscarriage, they did not have information on the specific conditions for which women were using NSAIDs.
It is possible that these medical complaints could still have confounded the association between the drugs and miscarriage. For example, important confounders that were not assessed, and that have been associated with increased risk of miscarriage, are various viral and bacterial infections, including sexually transmitted infections such as chlamydia.
Other possible confounders that were not assessed include lifestyle factors, such as smoking, alcohol consumption and body mass index. This was a complex study, which included many different comparisons. The British National Formulary currently states that NSAIDs should be avoided during pregnancy, unless the potential benefit is expected to outweigh the risks.
Other potential risks that have been associated with NSAIDs include delayed onset of labour and failed closure of the ductus arteriosus, which forms part of the foetal heart circulation. Aspirin should also be avoided due to the same risks, and because of its effects on platelet function, which increase bleeding risk.
